Privacy Notice

The Unfamous Ones (TUO) receives voluntary story submissions from healthcare professionals through our “Beyond the Uniform” form. This notice describes the information we request and how we intend to use it.

What we collect

When you submit the form, we collect the information you choose to provide, which may include your name, email address, phone number, preferred contact method, healthcare profession and experience, and descriptions of your personal and career journey. Our form service may also capture technical submission information.

Why we use it

We use submissions to evaluate possible TUO stories, contact applicants about potential interviews, administer the story-selection process, and maintain accurate internal submission records. A submission is not a promise of production and does not authorize publication of your story, name, likeness, or recordings. We will discuss separate releases and permissions before any production or publication.

How submissions are handled

The form is provided through Tally, and responses are transferred to TUO's connected Google Sheets account for internal review. Authorized TUO personnel and relevant service providers may process the data to operate the form. We do not offer story submissions for public browsing or sell submitted personal information. Please review Tally's privacy policy and Google's privacy policy for their handling of information.

Please do not submit patient or confidential information

This form is not designed to receive protected health information (PHI), patient-identifying details, medical records, patient photographs, or confidential employer documents. Please avoid names, precise dates and places, or unusual details that could identify patients. If a proposed story raises such issues, we will evaluate the appropriate permissions and privacy protections separately.

Retention and your choices

We retain submissions for as long as reasonably needed to evaluate story opportunities, manage follow-up, and meet applicable obligations. If you want to update, withdraw, access, or request deletion of a submission, contact us at stories@theunfamousones.com. We will review requests subject to applicable obligations and technical limitations.

Contact preferences

We use your contact details to follow up about your submission. If you choose text-message contact, we will honor the separate text-message consent included in the form. We do not use submission contact details for unrelated marketing without a separate basis or consent.
